In this module, you will examine the structure, function, and delivery of care in healthcare, public, and community health systems. You will learn about healthcare teams and your role as a CHW within the team. This module also focuses on developing your skills in providing care management. Finally, we will dive into the nuts and bolts of organization and documentation, which are essential to successful care of delivery as a CHW.

Learning objectives:

1. Collaborate with healthcare team members to provide care management.
2. Identify community assets and resources and coordinate services through outreach, networking, navigation skills, and referrals.
3. Use problem-solving strategies to make effective decisions.
4. Display knowledge and skills in work planning and organizing (time management and priority setting).

Audience: Youth 16⁺; Adults exploring healthcare careers.
